It is used to eliminate symptoms of disorders – swelling and pain along with inflammation that occur in the following cases:

  • when damage occurs in the area of soft tissues - injuries affecting tendons with muscles, and also ligaments with joints;
  • for bursitis;
  • in degenerative rheumatism of a local nature;
  • for tendinosis;
  • for rheumatism in the soft tissue area (localized);
  • for cervicobrachialgia;
  • with periarthropathy.

Forma de liberação

The release is carried out in the form of a patch, as well as gel and suppositories. Less common are such dosage forms as capsules with tablets and dragees, as well as a solution.

Olfen 100 rectocaps

Farmacodinâmica

The gel is used externally. The active ingredient penetrates the skin and accumulates inside the tissues at the treatment site. During the treatment of inflammation, the drug has an analgesic effect, reduces the severity of swelling, eliminates the inflammatory process, accelerates healing and cools the skin.

The patch is a product with a prolonged effect (maximum 12 hours). Due to its innovative form, it evenly distributes the medicinal component (diclofenac) on the area of the skin where it was applied. As with the gel form, the medicine penetrates through the skin into the tissues, relieves pain, and removes inflammation with swelling. At the same time, the drug has a cooling effect, which is very important for pain in the back and joints, as well as for sprains and bruises.

The other medicinal forms also have analgesic, anti-inflammatory and antipyretic properties. They have a positive effect on severe pain of inflammatory origin.

Farmacocinética

After applying the gel, the active component of the drug penetrates into the synovium with the synovial membrane, as well as into the blood plasma.

With plasma protein, the drug is synthesized almost 100%. The half-life of the substance from plasma is 1-2 hours.

Dosagem e administração

The drug is in gel form.

The volume of gel used depends on the size of the area to be applied. Generally, about 2-4 g of gel (approximately 1-2 cm) is required for a skin area of 400-800 cm 2. The medicine should be applied 3-4 times a day, it should be applied to the affected area, without rubbing it into the skin. A maximum of 15 g of gel can be used per day. It should be noted that the drug in the form of a gel can be used in combination with other dosage forms.

The duration of the course of use is determined by the type of disease, as well as the individual indications of the patient. It is recommended to evaluate the appropriateness of using the drug after 2 weeks of the course.

It is forbidden to use the gel for more than 14 days in a row in case of treatment of rheumatic pathology or soft tissue lesions. In case of pain caused by arthritis, it is necessary to use the drug for a maximum of 3 weeks (the only exceptions are cases when the doctor prescribes a different course duration).

If there is no improvement after 7 days of using the gel, you should stop the therapy and consult your doctor.

The drug is in the form of a patch.

Olfen in the form of a patch should be applied to the affected area 1-2 times a day. It is necessary to take into account that the skin in the area where the medicinal patch is applied should be completely dry and clean. It is prohibited to pre-treat the skin with cream. When applying the patch, do not stretch it. If it is necessary to treat an area with movable joints (for example, an elbow), it is recommended to additionally strengthen the patch using a mesh bandage.

Before performing hygiene procedures, the patch can be removed, after which it is glued back on.

Medicine in tablet form.

Children aged 6-12 years are recommended to be prescribed the drug in tablets (25 mg) with a standard duration of action. It is allowed to take 2 mg/kg of the drug per day. If juvenile rheumatoid arthritis is observed, the dosage is increased by 1 mg/kg. The tablet must be swallowed whole, washed down with liquid and without chewing. The intake is performed before eating.

Tablets with prolonged effect should be taken at a rate of 100 mg/day. If it is necessary to eliminate a migraine attack or dysmenorrhea, the drug should be taken at a rate of 200 mg/day.

Medicine in the form of a solution.

The solution is administered into the body by infusion. No more than 150 mg of the drug can be administered per day. Before the administration procedure, the drug must be diluted using a saline solution (0.9%) or a d-glucose solution (5%) - approximately 100-500 ml is required. Before dilution, a sodium bicarbonate solution is added to the drug: 1 ml (when using the 4.2% form) or 0.5 ml (when using the 8.4% form). Taking into account the severity of pain, the infusion must be carried out for 0.5-3 hours.

To eliminate pain that occurs after surgical operations, it is necessary to administer the solution in a dose of 25-50 mg over 15-60 minutes. Then the infusion should be carried out at a rate of 5 mg/hour until the maximum daily indicator (150 mg) is reached.

The drug can also be administered intramuscularly (150 mg once) – in acute forms of the disease or to prevent exacerbation of the chronic stage of the pathology. Further therapy is carried out using oral forms of the drug. The maximum daily dose remains the same – 150 mg.

The drug is available in suppository form.

Suppositories are administered rectally. To eliminate the initial symptoms of a migraine attack, 100 mg of the drug is required. If necessary, a similar dosage can be administered again.

Uso Olphena. durante a gravidez

The medicine is allowed to apply on the 1st and 2nd trimesters (with the permission of the attending doctor), if the possible benefit for a woman is more likely than the risk of complications in the fetus. In pregnancy, Olfen should be used in the least effective portions and for a short period of time. It is forbidden to apply the medicine on the 3rd trimester.

If the medicine needs to be used during the lactation period, you should first consult your doctor to determine if it will be necessary to stop breastfeeding.

Contra-indicações

Main contraindications:

  • intolerance to drug components;
  • history of acute rhinitis or urticaria, as well as attacks of bronchial asthma (provoked by the use of NSAIDs);
  • aggravated ulcer in the gastrointestinal tract;
  • the presence of burns, as well as eczema;
  • open wounds on the surface of the skin;
  • infections on the surface of the skin.

Side-effects are usually manifested in the form of transient local symptoms on the skin. Occasionally there is a pustular rash, allergic symptoms, bronchial asthma, Quincke edema, itching with swelling, increased sensitivity, photophobia and burning of the treated area of the skin. In addition, eczema with erythema, dermatitis and rashes may occur.

Interações com outras drogas

Olfen increases the plasma levels of lithium agents, methotrexate, and cyclosporine with digoxin. In addition, it weakens the effect of diuretics, and when combined with antiplatelet and thrombolytic drugs, as well as anticoagulants, it increases the likelihood of bleeding.

Combination with diuretic drugs (potassium-sparing) increases the risk of hyperkalemia. The drug weakens the effectiveness of sleeping pills, antidiabetic and antihypertensive drugs and increases the risk of developing side effects of GCS and other NSAIDs (most often, bleeding inside the gastrointestinal tract is noted). At the same time, the drug potentiates the toxic properties of methotrexate and the nephrotoxic properties of cyclosporine.

Combination with paracetamol increases the risk of developing nephrotoxic effects of the active component of Olfen. As a result of combination with ethyl alcohol, colchicine, corticotropin, as well as drugs containing St. John's wort, the likelihood of bleeding in the gastrointestinal tract increases.

Drugs that promote photosensitivity increase the sensitizing effect of the active component of the drug relative to ultraviolet radiation.

Drugs that block tubular secretion increase plasma levels of diclofenac, which is why its dosage will need to be adjusted when combined with these drugs.
